Every year, billions are spent in fertility clinics; the result of which is often in vitro fertilization (IVF). About 5 years ago, I began receiving emails from a nurse practitioner in Indiana who works in a fertility clinic. Her experience was dramatic; 5,000 IU/day for both the man and woman frequently resulted in a healthy baby. However, her last email to me was quite sad, she was in danger of losing her job as her boss, a gynecologist, was losing money due to vitamin D. He ordered her to stop advocating it or lose her job.

Today, the Daily Mail and several other newspapers reviewed a lengthy article in The European Journal of Endocrinology that concluded, “Given the high prevalence of infertility as well as vitamin D insufficiency in otherwise healthy young women and men and the possible role of vitamin D in human reproduction, research might lead to new therapeutic approaches such as vitamin D supplementation in the treatment of female and male reproductive disorders.”

Critical to this carefully caged advice is the fact that men need help as frequently as the women do. “Population-based studies found that in 30-40% of infertile couples the underlying cause is the male factor. In this context it should be mentioned that the overall semen quality of men is decreasing, which might partly be explained by environmental factors. Indeed, as much as 20% of young men have sperm concentration below the WHO recommendation level and 40% present with sperm concentrations below a level that is considered optimal for fertility.” Pretty amazing, especially when you realize these men have normal testosterone levels but that vitamin D levels are steadily decreasing.

The authors go onto say, “In northern countries, where a strong seasonal contrast in luminosity (sunshine intensity) exists, the conception rate is decreased during the dark winter months, whereas a peak in conception rate during summer leading to a maximum in birth rate in spring has been observed. Moreover, ovulation rates and endometrial receptivity seem to be reduced during long dark winters in northern countries.”

While no direct studies exist of vitamin D levels and fertility per se, the authors report, “In a study among 84 infertile women undergoing in vitro fertilization, women with higher levels of 25(OH)D in serum and follicular fluid were significantly more likely to achieve clinical pregnancy following in vitro fertilization . . .”

The takeaway message is the same as always, a message so common I should just start saying “ditto.” If you want to get pregnant, make sure you and your partner take 5,000 IU/day. New research suggests vitamin D supplements can reduce pain and may be a cost-effective alternative, or an adjunct to other treatment, for individuals with fibromyalgia syndrome (FMS).
The supplementation is helpful for fibromyalgia patients suffering from low vitamin D levels, say researchers in the journal PAIN®.

In addition to pain and fatigue, individuals diagnosed with FMS may experience sleep disorders, morning stiffness, poor concentration, and occasionally mild-to-severe mental symptoms such as anxiety or depression.

The condition can have a significant impact on the patient’s quality of life, resulting in loss of employment and/or withdrawal from social life.

There is no cure, and no treatment will address all of the symptoms, but some symptoms may be alleviated by physical therapy, cognitive behavioral therapy, temporary drug therapy (such as amitriptyline, duloxetine, or pregabaline) and multimodal therapies.

Calcifediol is a prehormone produced in the liver by the enzyme cholecalciferol (vitamin D3).
Calcifediol is then converted to calcitriol (1,25-(OH)2D3), which is the active form of vitamin D. The concentration of calcifediol in blood is considered the best indicator of vitamin D status.
Researchers hypothesized that vitamin D supplementation would reduce the degree of chronic pain experienced by FMS patients with low levels of calcifediol and also might improve other symptoms.

“Low blood levels of calcifediol are especially common in patients with severe pain and fibromyalgia. But although the role of calcifediol in the perception of chronic pain is a widely discussed subject, we lack clear evidence of the role of vitamin D supplementation in fibromyalgia patients,” said lead investigator Florian Wepner, M.D.

“We therefore set out to determine whether raising the calcifediol levels in these patients would alleviate pain and cause a general improvement in concomitant disorders.”
In a randomized controlled trial, 30 women with FMS with low serum calcifediol levels (below 32ng/ml) were randomized to a treatment or control group.

The goal for the treatment group was to achieve serum calcifediol levels between 32 and 48ng/ml for 20 weeks via oral cholecalciferol supplements.

Serum calcifediol levels were reevaluated after five and 13 weeks, and the dose was reviewed based on the results. The calcifediol levels were measured again 25 weeks after the start of the supplementation, at which time treatment was discontinued, and after a further 24 weeks without supplementation.

Twenty-four weeks after supplementation was stopped, a marked reduction in the level of perceived pain occurred in the treatment group.
Between the first and the 25th week on supplementation, the treatment group improved significantly on a scale of physical role functioning, while the placebo group remained unchanged.

The treatment group also scored significantly better on a Fibromalgia Impact Questionnaire (FIQ) on the question of “morning fatigue.” However, there were no significant alterations in depression or anxiety symptoms.

“We believe that the data presented in the present study are promising. FMS is a very extensive symptom complex that cannot be explained by a vitamin D deficiency alone.
“However, vitamin D supplementation may be regarded as a relatively safe and economical treatment for FMS patients and an extremely cost-effective alternative or adjunct to expensive pharmacological treatment as well as physical, behavioral, and multimodal therapies,” said Wepner.

“Vitamin D levels should be monitored regularly in FMS patients, especially in the winter season, and raised appropriately.”

The results may suggest that omega-3s protect the brain from the loss of volume that happens with normal aging and is seen more severely in people with dementia, the researchers say.
"The brain gets smaller during the normal aging process - about 0.5 percent per year after age 70, but dementia is associated with an accelerated and localized process of brain shrinkage," said James Pottala, who led the study.
Pottala is an assistant professor at the University of South Dakota Sanford School of Medicine in Sioux Falls and chief statistician for the Health Diagnostic Laboratory in Richmond, Virginia.
He and his colleagues analyzed data from the Women's Health Initiative Memory Study to see whether omega-3s were associated with brain shrinkage in general, and in specific brain regions involved in memory and other cognitive processes.
The data covered 1,111 women who were, on average, 70 years old and had no signs of dementia at the beginning of the study. At that time, the amounts of the omega-3 fatty acids eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) in their red blood cells were measured.
DHA accounts for 30 percent to 40 percent of the fatty acids found in brain cell membranes, and it's especially concentrated near the synapses where the cells communicate with one another, Pottala and his colleagues note in their report, published in the journal Neurology.
Red blood cell levels of the omega-3s are good indicators of how much a person has consumed, the researchers add.
The researchers used an omega-3 index to describe the fatty acid levels seen among women in the study and to divide them into four groups: women with the highest levels had an average index reading of around 7.5 percent, while women with the lowest levels had an average of 3.4 percent.
Eight years after the women's blood was tested, they underwent MRIs to measure the volume of gray matter and white matter in their brains.
The researchers found that women with the highest EPA and DHA blood levels at the study's outset had brains that were about two cubic centimeters larger overall than women with the lowest levels.
In addition, the hippocampus, a brain region critical to forming and storing memories, was 2.7 percent larger in women who had fatty acid levels twice as high as the average.
Of 13 specific brain regions the researchers looked at, the hippocampus was the only one where they saw a significant difference.
The analysis adjusted for other factors that could influence the women's brain size, including education, age, other health conditions, smoking and exercise.
The researchers didn't measure cognitive function, only brain volume, so they cannot say whether the size differences they saw had any link with differences in memory or dementia risk.
The authors acknowledged other limitations in their report, including that they did not look at whether the women's omega-3 consumption had changed over time.
It's possible that some of the participants had changed their diets or started taking fish oil or other forms of omega-3 fatty acids, Pottala told Reuters Health in an email.
But in previous study, he and his colleagues showed red blood cell EPA and DHA levels and peoples' dietary fish intakes generally don't change over time.
"If some subjects in our MRI study began taking fish oil supplements, then the reported benefits would be underestimated," Pottala said.
Pottala says higher blood levels of omega-3 fatty acids can be achieved by dietary changes, such as eating oily fish twice a week or taking fish oil supplements.
